Management of dandruff recurrency with a specific dermocosmetic shampoo

Dandruff is a chronic scalp disorder, characterized by white scales, pruritus and erythema. Desquamation of the scalp, with visible flakes loss, may lead to socio-affective impact. Anti-dandruff shampoo, is effective in resolving symptoms but with recurrence once stopped.
The aim was to investigate safety and efficacy of a specific dermocosmetic shampoo regarding dandruff reduction, its maintenance and anti-recurrence effect.
In a non-comparative study, on persistent dandruff or seborrheic dermatitis (SD), subjects used shampoo weekly 3 times from Day (D)0 to D21, then 1 to 2 times from D21 to D49. To evaluate the anti-recurrence effect, after D49±2 a mild shampoo was used in relay for 6 weeks. The clinical parameters severity on D0, D21, D49 and D91 and erythema intensity in SD were evaluated by a dermatologist. Scalp desquamation index was calculated from quantification of corneocytes on Corneofix. At D0, D21 and D49, subjects filled a questionnaire to assess efficacy. Tolerance of the product was evaluated.
Among the 36 subjects included (33 years on average, 86% female), 28 subjects were analyzed, 53% presented persistent dandruff, 47% SD with erythema. At D21, D49 and D91, a significant improvement was assessed for dandruff/SD severity (-53%, -66% and -60% respectively), pruritus severity (-71%, -80% and -76%, respectively), flakes/scales severity (-50%, 66% and -57%, respectively), desquamation index (-36%, -47% and -42%, respectively), and erythema for SD subjects (-50%, -72% and -74%, respectively) vs D0 (***p<0.001). For 96% subjects, the shampoo was overall pleasant, 71% to 100% soothed feeling of irritation, discomfort, and itching, decreased scalp redness, installed dandruff/scales and lastingly reduced reappearance. Product well tolerated (only one case of discomfort sensations product-related).
Anti-dandruff shampoo is efficient in severity reduction of dandruff, pruritus and erythema after 21 days of application, and in anti-recurrence with a long-lasting effect.
